Course Memo

Entrepreneurial Pharmacy and Practice Transformation will be a 1 credit elective for pharmacy students that meets for ten 2--hour sessions. This course is intended to be a practical look at incorporating the umbrella of opportunities related to implementing Medication Therapy Management (MTM), Chronic Care Management (CCM) and other clinical services in various pharmacy settings (example: Transitions of Care (TOC) with other members of the health care team). The goal is to provide “hands-on” exposure to what is involved in starting and demonstrating the pharmacists’ value for team-based care and/or collaborative arrangements with other clinicians. Students will learn to identify, develop and evaluate team-based care opportunities in different clinical settings. Students will complete a project with a partner(s). The project will require the “business partners” to design, develop and complete some of the steps necessary to develop a collaborative team-based model utilizing MTM and/or CCM in one of the various pharmacy settings.
